Dr. Tyler McElwee, a facial plastic surgeon based in Houston, explored the intricacies of blepharoplasty or eyelid lifts in his Instagram weekly series, “Mondays with McElwee.” He emphasized that upper blepharoplasty primarily involved removing excess skin, sometimes accompanied by underlying fat. The incision, discreetly placed within the natural upper eyelid crease, resulted in minimal scarring, negligible discomfort, and swift recovery.
He mentioned in the Instagram video posted on the 26th of September (Episode 4), that lower eyelid procedures were more multifaceted, occasionally addressing underlying fat. Dr. McElwee described an internal eyelid incision, unseen but effective in fat adjustment or removal, reducing under-eye puffiness.
“Upper blepharoplasty removes excess skin discreetly, healing immaculately, while lower eyelid procedures address fat, aiming for scarless adjustments. Eyes hold the key to a youthful appearance, and I’m excited to answer more queries.”

“What is a blepharoplasty or an eyelid lift?”
An upper blepharoplasty basically involves removing a small amount of excess skin, sometimes along with a bit of underlying fat. However, typically, it’s just the excess skin that’s excised. The incision is completely hidden within the natural crease of the upper eyelid. It heals immaculately, causing very little pain and ensuring an easy recovery.
The lower eyelid procedure is slightly more complex. Sometimes, it involves addressing the underlying fat as well. For this, we make a small incision on the inside of the eyelid, which remains unseen. This approach is effectively scarless since the incision is not visible. We can adjust the fat by either removing a small amount of excess or repositioning it to reduce the appearance of under-eye bags. Occasionally, a tiny incision just below the eyelid might be necessary to address slight slackness. However, this is determined on a case-by-case basis, which requires an individual consultation.
Eyes are often the focal point when observing someone’s face. Refreshing and rejuvenating their appearance can significantly contribute to a more youthful look. I’m eager to address more questions from everyone.

Immediate aspects of recovery include edema (swelling), possible bruising, and minor discomfort or soreness. Your surgeon may prescribe analgesics to help with any discomfort. Keeping your head elevated and not partaking in strenuous activities will help improve the edema.
